Complete the required fields.
If you want to authorize a Direct Deposit or an Automatic Payment,
complete the appropriate depositor or payee information.
Print out what you have completed.
Drop the information off at any Midstates Bank location.
Sign letter(s) of transfer authorizations while you’re there.
