$50 Visa® Gift Card Sweepstakes Official Rules 2023

Complete the emailed survey within 48 hours of receiving the survey request email for a chance to win one of two $50 VISA® Gift Cards. Two winners will be selected from all eligible entries. Promotion period is between August 14, 2023 and September 8, 2023.

Rule & Regulations

1) TO ENTER: There are two (2) ways to enter the contest during the promotion period:

1. Automatic entry: Every individual who completes the survey, including their name, within the time frame specified will be automatically entered. Limit one entry per person. Certain restrictions may apply.
2. To enter without purchase: Send a 3” x 5” postcard with your name, address, phone number and email address to: Midstates Bank $100 Visa Gift Card Giveaway, c/o Marketing Department, P.O. Box 717, Harlan, IA 51537. Mail-in entries must be postmarked by September 8, 2023 and received by September 15, 2023 in order to be eligible. Limit one mailed entry per person. Certain restrictions may apply.

Entrants may enter only by using one of the entry methods described in these rules. Entries that are mechanically reproduced, copied, illegible, incomplete, or inaccurate are void. Entries become the property of Midstates Bank N.A. and will not be acknowledged or returned.

2) SPONSOR: Midstates Bank N.A., 812 Durant St, Harlan, IA 51537

3) PRIZE: Two (2) VISA® Gift Card, retail value of $50.00. Two (2) prize winners will be drawn from all qualified entries received. Winners may make no substitution/transfer of prize. Winners are responsible for all other expenses associated with receipt or use of prize.

4) WINNER SELECTION AND NOTIFICATION, AWARDING OF PRIZE: Two (2) winners will be selected by Midstates Bank N.A. by randomized selection from among all qualified entries received by close of business September 15, 2023. Winner need not be present to win. Drawing will be held the week of September 18, 2023. Decisions of Midstates Bank N.A. are final. Win­ners will be notified by phone on or about September 18, 2023. If a selected winner is ineligible, declines the prize, or if Midstates Bank N.A. unable to contact a selected winner after reasonable effort and time (7 days), the prize shall be forfeited and will be awarded to an alternate winner selected by another randomized selection from all remaining eligible entries. The winner must claim the prize via instructions provided by Midstates Bank N.A. Sponsor will determine specific prize components in Sponsor’s sole discretion. Winner is responsible for all taxes on prizes and expenses, including, without limitation, federal, state, and local sales, and income taxes, luxury taxes and any other costs incurred in claiming or using the prize. No prize substitutions or cash equivalent of prize is permitted.

5) ELIGIBILITY: Sweepstakes is open to legal U.S. residents, 18 years of age and older at the time of entry, who are not employees of Midstates Bank N.A. or members of their immediate household or immediate family. No purchase necessary. Purchase does not improve chances of winning. Odds of winning depend on total number of qualified entries received. Only one entry per person. See any Midstates Bank location for official rules. Void where prohibited.

6) PRIZE RESTRICTIONS: Prizes are non-transferable. The winner may receive an IRS Form 1099 from Midstates Bank N.A. for the value of the prize. Federal, state, and local taxes, and any expenses relating to the acceptance and use of the prize not specified, are the sole responsibility of the winner. Limit one prize per person.

7) ODDS: Odds of being selected as a sweepstakes winner will depend on the total number of eligible entries received.

8) ADDITIONAL TERMS: By participating, entrants agree to (a) The Official Rules and the decision of the employees of Midstates Bank N.A., which shall be final in all respects; (b) release, discharge and hold harmless, Midstates Bank N.A., their respective parents, subsidiaries, affiliates, officers, directors, and employees from any liability, claims, or damages arising out of their participation in the Sweepstakes, and the acceptance, use, misuse, or possession of any prize. By accepting a prize, the winner agrees to, where legal, the use of their name and/or likenesses for advertising and publicity purposes without additional compensation or consideration.

9) LIMITATIONS OF LIABILITY: Midstates Bank N.A. is not responsible for the printing of or typographical errors in any Sweepstakes-related materials; for stolen, lost, late, misdirected, damaged, incomplete, illegible, or postage-due mail or entries; or for transactions that are processed late or incorrectly or are lost due to computer or electronic malfunction. Midstates Bank reserves the right to cancel or modify the Sweepstakes if fraud or technical failures compromise the integrity of the Sweepstakes as determined by Midstates Bank in its sole discretion.

10) CANCELLATION: If for any reason beyond the control of Midstates Bank N.A. the Sweepstakes is unable to be held, or for any cause affecting the Sweepstakes’ operability, fairness, integrity, or accuracy, Midstates Bank N.A. reserves the right to modify, suspend, or cancel the Sweepstakes without notice.
